Before Getting Started

A link to the online application form is provided belowPlease read all of the information provided on this page and on the Eligibility and Criteria page before completing your application. Listed below are the documents required for application. These documents, listed 1 through 6, will constitute your application packet.


1.  Application Form

A link to the online application form is provided below. Be sure to follow all instructions provided here and in the application form. If the Scholarship Application Form link below is not active, the contents of this page are not current.

This form cannot be saved for retrieval at a later time, so you should not begin entering data until you can devote sufficient time to completing the entire form in one sitting. You might find it helpful to print a blank copy of the form to complete by hand as time permits, and then use your handwritten "rough draft" to make completing the form online faster and easier.

Before getting started, create a text file (Word, WordPerfect, etc.) to contain your work history, extracurricular history, personal statements, and essays — then copy/paste to the appropriate fields in the application. This will allow you to have access to the information for other scholarship applications as well as give you the benefit of proofing tools available with most word-processing software. +++++++++++++++++++++++++++++++++++++++++++++++++++++++++++

2.  Agreement Statement

The Agreement Statement is designed to be printed, read carefully, and then signed as appropriate. A link to the Agreement Statement is provided below.

3.  FAFSA4caster

FAFSA4caster is a tool designed by the U.S. Department of Education to provide an early estimation of a student's eligibility for financial aid. Clicking the link below will take you to the FAFSA4caster website, where you will be prompted for estimated personal and financial information relative to your family's circumstances. When data entry is complete, you will arrive at a page containing your Estimated Family Contribution (EFC) number. Print this page to be included with your application packet. It is strongly recommended that you use the Print Preview feature to be sure that the page you print includes your EFC number.

Remember: Print only the page that includes your EFC number (write your name on the page).

4.  Essays

Submit two typed essays of about 600 words each (no more than one 8.5x11 page each), addressing the questions listed below. Include your name and the essay question as the heading for each page.

What obstacles have you or will you have to overcome to attain your educational goals?

Essay Two
What are your plans to serve your community now and in the future?

 5.  Letters of Recommendation

Submit two letters of recommendation enclosed in sealed envelopes. One letter should be from a teacher or high school counselor. The other should be from someone — not a school official and not a Community Foundation board member and not a relative — who can attest to your volunteer work and community service participation, leadership abilities and/or religious activities.

6.  Official Grade Transcript

Your official grade transcript will be added to your application packet by your high school guidance counselor.

Your application packet must be delivered to your high school guidance counselor before the school day ends on January 11, 2012.
